Dear Madam:

We are in receipt of your June and July premiums but inasmuch as June was not paid within the period of grace, before we can accept these premiums now, it will be necessary for you to sign the enclosed Certificate of Good Health while you are in good health and return it to us by return mail.

Immediately upon receipt of this form signed as requested, your receipt card, properly credited will be returned to you, and your policy will be revived accordingly.
